The following information will be provided as a look-up based on the Investigator’s name: 1) Investigator’s name, 2) Investigator’s title and role on the PHS research project, 3) Name of the entity in which the significant financial interest (SFI) is held, 4) Nature of the SFI, and 5) Approximate dollar value of the SFI.
The UW-Madison must update the website annually and within 60 days of identifying any new FCOIs. The UW-Madison must retain information on the website for a minimum of three years from the date that the information was most recently updated.
