Conflict of Interest Committee
Updating Outside Activities Reports
If you have major changes in your outside activities between annual reports (e.g. new ownership interests in a company or new involvement in human subjects research with an entity that you consult for), you must update your report using the following procedure. You may also use this procedure to make any necessary changes in your annual report.

Contact the Conflict of Interest Committee staff and inform them that you wish to update your outside activities report.

The COI Committee staff will send you email telling you that your most recently submitted report may be revised.
- You will then be able to access your report by opening the reporting
tool (https://www.gradsch.wisc.edu/disclose/)
and logging in with your NetID.

You must resubmit your report when you have finished making changes by using the "Submit My Completed Report" button on the Main Menu page of the reporting program.
You will be asked to complete your revisions within two weeks of receiving email from the COI Committee staff, to avoid a long lapse in your report. The email will also include instructions for notifying the individuals who reviewed your report that new report information needs review.
