Print a Human Subjects Training Completion Certificate
CITI Human Subjects Training
To verify or print a record that you have completed the UW-Madison CITI Human Subjects Protection Training:
Log into the UW-Madison Researcher Certificate of Completion Database (this requires UW-Madison NetID authentication).

Type in the last name of the person you are looking up. If you get a message "too many records," hit the space bar once and then start typing the first name.

Click on the name.

Print the Completion Certificate using your browser's print feature.

The Certificate of Completion will have the completion and expiration dates
To print a copy of a completion certificate from CITI:

Log into CITI (How do I access the CITI course?).

From the "Learners Menu" page, click on the "Previous Coursework Completed" link.

The "Completion Reports" page will list all the CITI courses you have completed.

Click "Print Completion Report" for the training for which you want to print a Certificate of Completion.
Previous UW-Madison Human Subjects Tutorial
As of January 1, 2009, certificates of completion from the "old" UW-Madison Human Subjects Tutorial are no longer accepted by the UW-Madison IRB Offices as proof of human subjects training for purposes of protocol submission.
Instead, all researchers and research staff engaged in human subjects research on a UW-Madison protocol must take the new UW-Madison CITI Human Subjects Protection Training.
